Application Information for
NACTEL Associate Degree Graduates

If you graduated from the NACTEL Program with an Associate’s degree, you will need to complete and submit the following 2 forms:

• Application for Resumption of Studies Form
• Application for Change of Major Form

These forms can be found on the Forms page in the Student Information Center . Please note that you will need Adobe Acrobat Reader installed on your computer to access the required forms online. To download Acrobat for free, click here.

Once you’ve accessed and completed the forms:

1. Fax the completed and signed Application for Resumption of Studies Form and the Application for Change of Major to: 1-914-989-8616
2. The forms will then be reviewed to be sure you are in good academic and financial standing.
3. Once your program has been changed over to the Bachelor's Degree, you will be sent an advisement e-mail notifying you of this change and your next steps for registering.
